“Broken fax then noted the wrong dose”

“An elderly woman died on a short-term accommodation in Gothenburg at the end of October last year, after they gave her the wrong dose of medicine. The sequence of events which led to the death notification now to the Inspection for health care (Ivo), writes Göteborgs-Posten.”
